Scheduling & Client Forms
I AM ONLY SCHEDULING TELEHEALTH / TELETHERAPY SESSIONS
To schedule your first visit, please go to the "Contact Me" page and complete the Intake Form. Contact via my website and email is preferred as often times I am unable to answer my phone as I am usually in session with another client. If I am unable to answer, please leave a detailed message and I will return your call within 2 business days. When you schedule your first appointment, the intake paperwork will be emailed to you to complete before your appointment. You will need to have the forms completed the day prior to your first appointment or else your appointment will be cancelled.
If the new client is a teen that will be coming in for individual counseling, please have him/her complete the intake form themselves (specifically the symptoms checklist portion).
Also due prior to your first visit will be a copy of your insurance card, a photo identification, custody paperwork (if appropriate), and a credit card to be kept on file (please see "Insurance & Fees" section for more information). The online intake form allows you to upload your photo ID and insurance card. Custody or other paperwork will need to be emailed separately. You will be able to see a credit card on file after your first session when you are sent an invoice from Square.
All my forms are available for completion online, including sending paperwork. A link will be sent prior to your first appointment.
