Insurance and Fees

IF YOU ARE LOCATED IN CALIFORNIA: ONLY TELEHEALTH SESSIONS ARE AVAILABLE AT THIS TIME. YOU MUST CHECK WITH YOUR INSURANCE COMPANY TO SEE IF THEY COVER TELEHEALTH / TELETHERAPY SESSIONS.  My license requires that for Teletherapy you must be located in either the State of California or the State of Idaho at the time of the session.  Some insurances require a certain Telehealth system to be used but at this time I am only using the system Doxy.me which is HIPAA Compliant but some particular insurances may not allow teletherapy on a system other than their own.  I have a form available for you to use when calling your insurance company to help you ask the correct questions about your coverage.

FOR CALIFORNIA CLIENTS: I am currently accepting the following insurance plans:  Anthem Blue Cross, Blue Shield, HealthNet, MHN, Cigna, Aetna (most plan only cover Teletherapy through TeleDoc which I am not able to use), Magellan, United Health Care (United Behavioral Health), Tri-Care, E4Health EAP, WellCall EAP and IEHP (they are working on adding Telehealth coverage).  

 

IF YOU ARE LOCATED IN IDAHO: I HAVE TELETHERAPY SESSIONS AVAILABLE FOR THE ENTIRE STATE OF IDAHO AND AN OFFICE LOCATION IN BOISE, IDAHO.  If you choose teletherapy, some insurances require a certain Telehealth system to be used but at this time I am only using the system Doxy.me which is HIPAA Compliant but some particular insurances may not allow teletherapy on a system other than their own.  I have a form available for you to use when calling your insurance company to help you ask the correct questions about your coverage.

FOR IDAHO CLIENTS: I am currently accepting the following insurance plans: Idaho Medicaid / Optum Idaho, Tri-Care, Regence Blue Shield, Idaho Blue Cross and IPN.  I am also in the process of becoming in-network with Mountain Health Co-Op, Select Health and St. Luke's Health Partners so please check back soon.

 

If I am not a preferred provider for your insurance company, I can provide you with a SuperBill which allows you to seek partial reimbursement from your insurance company using your Out of Network Benefits.  You can call your insurance company to find out if you have Out of Network benefits and what the reimbursement amount would be.  The way a SuperBill works is that you would be seeing me as a cash-pay client and I will provide you with a SuperBill showing that you paid for a session.  You would then submit the SuperBill directly to your insurance company and they will then reimburse you directly at the Out of Network rate that is available on your plan.  I cannot verify your Out of Network benefits for you, so you will need to call your insurance company directly and inquire about this option if you plan to utilize your Out of Network benefits.  Most clients report getting reimbursed in approximately 1-2 weeks.  

 

If I am an In Network Provider for your insurance company, your benefits will need to be verified prior to your first visit, so please call ahead of time with your insurance information readily available or complete the New Client Intake Form on the "Contact Me" page.  

 

For non-insurance clients, my cash-pay rates are as follows: Initial Intake Session $160 and all sessions thereafter are $135.  Sessions are approximately 45-50 minutes.

 

For any written report that is needed, the fee is $50 per page.  This fee is to be paid in advance.

 

For any court appearances the rate is $1500 per half day and $2500 for a full day.  This fee is to be paid in advance and requires at least a 4 week notice to allow ample time for me to make time in my schedule.  

 

For any court cases requiring a phone consultation with an attorney, mediator, etc., the fee is $200 per hour charged in increments of 15 minutes.

 

Appointments cancelled with less than a 24-hour notice or missed appointments are charged at my cash-pay rate and each client will be required to have a credit card kept on file in order to process this fee should this occur.  

 

I prefer payment in the form of cash but am able to accept credit card payments (Visa or Mastercard).  All payments and co-pays are due at the time of service. For teletherapy sessions, an invoice will be sent via email and is to be paid within 24 hours or you may keep a credit card on file for your session fees.

 

All sessions are 45-50 minutes in length.  Online intake forms will be emailed to you at the time of scheduling your first appointment and are required to be completed the day prior to your first appointment.  If the online intake forms are not completed the day prior to your appointment, then your appointment will be cancelled.
