Scheduling & Client Forms

To schedule your first visit, please go to the "New Client Request" page located under the "Contact Me" tab or click the button above. Contact via my website and email is highly preferred as often times I am unable to answer my phone as I am usually in session with another client.  If you choose to call by phone and I am unable to answer, please leave a detailed message and I will return your call.  When you schedule your first appointment, the intake paperwork will be emailed to you from IntakeQ to be completed before your appointment.  You will need to have the forms completed the day prior to your first appointment or else your appointment will be cancelled.  

 

If the new client is a teen that will be coming in for individual counseling, please have them complete the intake form themselves (specifically the symptoms checklist portion).

 

The intake paperwork will require a photo identification to be uploaded and a photo of your insurance card if you are using insurance benefits. Any custody paperwork or other relevant paperwork will need to be emailed separately from the intake forms.  You will be able to put a credit card on file after your first session when you are sent an invoice from Square.

If you are scheduling a Telehealth appointment, the online link and instructions will be emailed to you once your intake paperwork is completed prior to your first session.  The online link remains the same for every session so please save that email or bookmark the website on your internet browser.
